Shared Questionnaire System

I modified the simple example form, printed the PDF, marked the bubbles with pen, scanned to TIFF and processed. The results are that the 3rd bubble for each question is not being found in the right place, and none of the darkened bubbles are being recognized. Am I making a basic mistake?
